Carrier Packet Checklist

This checklist covers everything a new carrier needs in order to legally operate and set up with freight brokers. Work through it in sections — authority verification must come first.

Section 1: Authority Verification

USDOT number confirmed Active in FMCSA SAFER system
MC Operating Authority confirmed Active in FMCSA SAFER system
BOC-3 confirmed on file in FMCSA LMIA
Insurance filings confirmed on file in FMCSA LMIA

Section 2: Broker Setup Documents

Certificate of Insurance (COI) — current policy dates
W-9 (signed, current EIN and business name)
Voided check or bank letter for ACH payment
CDL copy — front and back
Operating Authority letter or certificate
Owner/contact government-issued ID

Section 3: Equipment & Compliance

ELD installed and operational
Vehicle Annual Inspection current
USDOT number displayed on both sides of cab
IRP (apportioned) plates current
IFTA decals applied to qualified motor vehicles
