Verify at safer.fmcsa.dot.gov before contacting any broker.

Must show current policy dates, coverage amounts, and named insured matching your carrier name. Most brokers require $1M liability and $100K cargo minimum.

Download from IRS.gov. Fill with your business name, address, and EIN.

Needed for direct deposit payment from the broker.

Notes on Each Item
- COI (Certificate of Insurance)
- Request this from your insurance agent. It should be updated each time your policy renews. Keep a PDF copy on hand — you'll send this to every broker you set up with.
- W-9
- A single, signed W-9 can be reused with all brokers until your information changes. Re-sign if you change your business name, EIN, or address.
- Carrier Agreement
- Every broker has their own version. Read the payment terms section carefully. Standard is 30–45 days. Some offer quick pay at 2–3 days for a fee of 1–3%. Note any requirements about COI updates, insurance minimums, and safety score thresholds.
